Since ancient times, leech therapy has been one of the best parasurgical treatments used all over the world. Leech therapy was a good treatment for blood purification, which eliminated many blood-borne diseases. It was previously thought that the benefits of leech therapy came from the amount of blood that was lost, but later studies have shown that more than the blood loss, it is the leech's saliva secretions, which contain an amazing combination of medicinal substances that are found in many medical and surgical conditions have therapeutic uses. Leech extract was discovered in the late 19th century and since then many medicinal products have been developed using leech extract for various diseases, but some of them have been abandoned. Leech extract is a valuable source of bioactive substances that can be used to treat mild and life-threatening medical disorders. The purpose of this review is to discuss the development of leech extract as a therapeutic and medicinal agent regarding the pain relief properties of arthritis, anticancer properties and the role of its active proteins in the wound healing process
